I said Wow! after I finished reading this.
Nice image.
Good universal questions that we all ask. You presented it well here in your poem.
I like your first part that ends with moonlight.
And
your last 5 lines.
One suggestion: you might want to break this up into stanzas by putting empty lines in between. It would make it look more like a poem on the page. And it would be easier to read. (just a thought, you know best, it's your work.)

The winds of change blow within, around and through everything, even relationships. Doubt and sincere questions develop and ride the currents with every gust of wind. This is a very poignant write. I especially liked the last three lines ---
'When I fall will you extend your inner spirit to catch my worthlessness,
Or cast me aside to be forgotten,
Only to replenish yourself with the next winds of change'

'Winds of Change' has the tenor of one who is uncertain of his ability to maintain a loving relationship. The winds of change have the capacity to destroy.
