I could be mistaken...but I don't think you're suppose to put names by the discussion. The idea is that the reader can tell each person apart by what you write.

If you look at the examples I do not believe it has names next to the words...

Examples are just examples, not commandments or even instructions. Do the instructions state "no names"?
